Application Instructions for CSHS Clinics

 

 

How do I Apply?

 

You can apply for CSHS services in one of three ways. 

 

1. Mail the paper application to CSHS.

 

Click here to download a CSHS Application and mail the filled out application to your nearby CSHS office.  Please note: So that the application process goes smoothly, please be sure you have included one of the following three things when you mail in the application:

  • Please have your child's medical provider (doctor, nurse practitioner, audiologist) fill out and sign Section 5 (Diagnosis Confirmation) of the application; 

OR

  • Please have your child's medical provider (doctor, nurse practitioner, audiologist) give you a note with your child's diagnosis written on it;

 OR

  • Please have a copy of your child's medical records that contains information about your child's diagnosis.

 

 

2. Have someone at a Parish Health Unit help you to fill out the application.

Call the parish health unit in the parish you live in, and make an appointment to fill out the CSHS Application. If you do not know where your local parish health unit is, click here Please note: You will need to bring to the appointment your child's medical information that describes your child's diagnosis. Please bring either a note from your child's medical provider (doctor, nurse practitioner, audiologist) or a copy of your child's medical records.

 

3. Fill out an application in person with staff from CSHS.

Call the CSHS regional office near where you live to make an appointment to fill out the CSHS Application; click here to find your nearby CSHS office. Please note: You will need to bring to the appointment your child's medical information that describes your child's diagnosis. Please bring either a note from your child's medical provider (doctor, nurse practitioner, audiologist) or a copy of your child's medical records.
