Service Results for Aging Adults/Senior Citizens

This application is for Louisiana certified nursing homes (or management companies who operate certified nursing homes in Louisiana) who wish to compete for a limited number of awards up to $19,500 to carry out projects that will make their nursing home a better place to live, visit, or work.

The Office of Aging and Adult Services (OAAS) was formed within the Department of Health as a health care reform initiative, bringing together all of the long-term care programs that serve senior citizens and people with adult-onset disabilities.

Adult Protective Services is here to help. APS investigates to assess the adult's risk of harm. If APS concludes that the adult is at risk, it works to arrange for services to protect the adult from further harm. APS strives to help individuals live free from abuse in a manner that supports choice and independence.

MDS-HC assessment and Care Planning trainings are offered only to Louisiana Support Coordinators and Assessors who currently work for a Support Coordination Agency, PACE, or Long Term Access Contractor that provide services to individuals receiving Community Choices Waiver, Adult Day Health Care Waiver, PACE, and/or LT-PCS.

A nursing facility provides 24-hour care for rehabilitative, restorative and ongoing skilled-nursing care to patients or residents in need of assistance with activities of daily living such as bathing, dressing, transferring, toileting and eating. Learn more.

Individuals that are elderly and adults with physical disabilities – elderly, seniors (age 65 and older) or adults with a physical disability (who developed the disability at age 22 or later) residing in nursing facilities (NF) who meet My Place Louisiana program eligibility requirements can receive My Place Louisiana assistance to transition from a qualified institution to a home and community-based living setting of the individual’s choice.
