Export Certificates and Certificates of Free Sale

 

SPECIAL NOTE: If you do not hold a Permit to Operate or a Certificate of Registration with the Louisiana Department of Health, we will not be able to issue you a certificate. These certificates are intended to address firms and products that are inspected by or registered with this office. Raw agricultural commodities, pesticides, medical devices, pet foods and animal feeds, and general consumer commodities are among the categories of products for which we cannot issue certificates.

If you are a manufacturer in the state of Louisiana exporting food, drug, or cosmetic products outside of the U.S., the recipient country's regulations may require that you have one or more of the following documents accompanying your shipment(s):

Note: either of the above may be used in multiple iterations if additional pages are required (for numerous products).

  • Certificate of Free Sale - This certificate stipulates that your firm is permitted in the state of Louisiana and has products registered with FDU. Furthermore, it states that your facility is in compliance with cGMP, as delineated in Part VI of Title 51, L.A.C.
  • Certificate of Free Sale (Bilingual - Spanish) - This certificate stipulates that your firm is permitted in the state of Louisiana and has products registered with FDU. Furthermore, it states that your facility is in compliance with cGMP, as delineated in Part VI of Title 51, L.A.C. This version includes a Spanish text translation.
  • Certificate of Free Sale (Bilingual - Simplified Chinese) - This certificate stipulates that your firm is permitted in the state of Louisiana and has products registered with FDU. Furthermore, it states that your facility is in compliance with cGMP, as delineated in Part VI of Title 51, L.A.C. This version includes a Simplified Chinese text translation.

The process for completing these certificates is as follows: download the appropriate form to your computer, load the form into your Adobe Acrobat or Adobe Acrobat Reader software and fill in the empty fields with the requisite information.

Do not leave any fields blank.

If the document asks for your registration number, this is the number on the FD-8 Certificate of Registration issued by this office to your firm. If the document asks for your permit number, this is the complete permit number provided on your firm's Permit to Operate issued by the Department of Health.

If you do not have either a registration or a permit, it is unlikely that we will be able to issue a certificate to you (see the SPECIAL NOTE above). Save a copy of the document with all fields completed to your computer's hard drive. Email it to Talacey.Williams@la.gov. She will make the requisite number of hard copies on cardstock and the document(s) will be signed by our Program Administrator or Chief Sanitarian for Specialty Operations (and LDH Legal Notary Public, if necessary) and mailed back to you at your return address.
