What you may need to apply:

  • Social Security Numbers (or document numbers for any legal immigrants who need insurance)
  • Employer and income information for everyone in your family (for example, from paystubs, W-2 forms, or wage and tax statements)
  • Policy numbers for any current health insurance
  • Information about any job-related health insurance available to your family
  • Resource information (for example, bank accounts, stocks, vehicles, life insurance)*
    • *Most people won't need this

Do I Qualify for Medicaid?

To see if you qualify for Medicaid, we use the Federal Poverty Level income guidelines. You can check the Monthly Income Limits for Medicaid Programs for more details.

In Louisiana, you may qualify for Medicaid if you:

  • Receive Supplemental Security Income (SSI) from the Social Security Administration (SSA).

You might also qualify if you:

  • Are disabled according to SSA’s definition.
  • Have corrected vision no better than 20/200.
  • Are a low-income parent of children under age 19.
  • Are a child under age 19.
  • Are pregnant.
  • Have no insurance and need treatment for breast and/or cervical cancer.
  • Receive Medicare coverage and are low-income.
  • Are aged 19 to 64 years old, have a household income less than 138% of the federal poverty level, don’t already qualify for Medicaid or Medicare, and meet citizenship requirements.
  • Get financial help from the Office of Family Support (OFS) through the Family Independence Temporary Assistance Program (FITAP)

At a Local Application Center

There are application centers across the state.  Some have rules about when they can assist, and many require an appointment.  Be sure to call ahead.

At a Regional Medicaid Office

Regional Medicaid offices are open Monday - Friday from 8 am to 4:30 pm.
