Close Your Medicaid

You have options.  You can request closure...

The easiest way to close your Medicaid is to log into your account in Medicaid's Self-Service Portal at MyMedicaid.la.gov.

It takes four steps and no more than five minutes to close your Medicaid.

• When you log into the Self-Service Portal, select the option "Request closure of your case" and select "yes" to confirm you want to close your case.
• You will be asked to review your household information and sign with an electronic signature to confirm your closure.
• You will get a confirmation number when the process is complete.

Complete this form, sign and return to Medicaid.

Or write Medicaid a letter that includes: 

1. Your name and either your Social Security number or your Medicaid Case ID number,
2. The name and date of birth for everyone in the case you want to close
3. Your signature

What happens next?

After you tell us you want to close your Medicaid, we will process your request.
