  Managed care entities (MCOs, PIHPs, PAHPs, and HIOs) are required to disclose information on ownership and control of the organization upon the managed care entity submitting a proposal in accordance with the State's procurement process; upon the managed care entity executing a contract with the State.; upon renewal or extension of a contract; and within 35 days after any change in ownership of the managed care entity.
