STI Reporting
- Chlamydia
- Gonorrhea
- Syphilis
- Chanchroid
- Granuloma inguinale
- Herpes simplex virus - neonates only
- Lymphogranuloma venereum (Trichomoniasis infection currently is not a reportable condition in Louisiana)
Electronic - STIs and HIV can be reported electronically through the Infectious Disease Reporting Information System (IDRIS), operated by the Infectious Disease Epidemiology Program. For information on setting up IDRIS reporting at your facility, please call 504-568-8313.
Mail/Fax - STIs may be reported using the "Louisiana Department of Health Confidential Report of Sexually Transmitted Infections (STI)" form (STI 43). Complete and mail or fax to the contact below.
Phone - Reports of early syphilis cases should be made by phone at the number below or your regional STI Program staff.

Hepatitis B and C Reporting
